Enrollment Support

Upon completion of a client’s plan design, Universal Rx develops a Members Communication Packet providing comprehensive, customized materials.

There are five (5) pieces that make up this packet:

  1. Benefit Outline:
The Benefit Outline is a reference guide providing a detailed description of retail, mail order, and any other optional benefits such as the diabetes management program and Expatriate Prescription Services benefits. This includes co-payments, dispensing limitations and covered/non-covered therapeutic categories.
  2. Enrollment Session
  3. DrugSource’s Mail Order Pharmacy Brochure and Enrollment Form:
This is a brochure specifically designed by DrugSource and provides information pertaining to the mail order program. This brochure also provides information on how to enroll and reorder.
  4. Diabetes Management Program Brochure and Enrollment Form:
This brochure is specifically designed by Liberty Medical providing Universal Rx plan members with information pertaining to the diabetes management program. This brochure provides a toll free telephone number which has been designated solely for Universal Rx plan members to call. This designated line is answered by a team of specialized representatives who manage all aspects of enrollment, ordering and reordering for Universal Rx plan members only.
  5. Formulary List:
The Formulary List is provided as a reference guide and quick indicator outlining which tier medications fall into. This educates and informs plan members as to what their co-payment will be before they go to the pharmacy.
  6. Expatriate Prescription Services Member Brochure and Order Form:
The EPS Member Brochure is designed to provide information on how the program works. It also clearly outlines how to enroll into the program and place orders. The EPS Order Form is customized specifically to each client's plan design so plan members can reference plan specific information if necessary.
